A New Trick for the Venotomy in Neonates
نویسندگان
چکیده
The management of Low Birth Weight (LBW) and Very Low Birth Weight (VLBW) neonates needing intensive cures usually requires an intravenous line for total parenteral nutrition (TPN), blood and platelet transfusions, antibiotics administration and blood samplings. In this respect, the use of central venous catheters (CVC) has become mandatory in neonatal intensive care units (NICUs) [1-2]. Peripherally Inserted Central Catheters (PICCs) are the most common used device by the neonatologists facing LBW and VLBW neonates [2-3]. When these patients develop surgical complications or need of high volume liquid infusion or exchange blood transfusions, or multi drugs administration, percutaneous ultrasound-guided CVC placement through the jugular, subclavian or right brachiocephalic vein represents the treatment of choice [1-3]. However, the experience with percutaneous CVC placement in VLBW newborns is limited. When large CVC are needed, their surgical placement still remains a common procedure in NICUs [4]. For its superficial position and satisfying caliber, saphenous vein is one of the most common used vein [4]. Because of dissection of the surrounding tissue, once identified and exposed, saphenous vein usually collapses that makes its cutdown very challenging, particularly in these very small babies. With scissors kept oblique or perpendicular to vessel’s course, an opening must be create wide enough to allow a smooth introduction of the catheter or at least the insertion of one of the scissor jaws to open the lumen with a second longitudinal cut to the intima. Damaging these very tiny and fragile veins, even up to the extent of cutting the posterior wall, encumbers this step of the procedure. As these complications are not uncommon, we conceived a new trick to safely cutdown these veins, successfully performed in more than 300 procedures in the last 20 years.

LATE EMBOLECTOMY FOR LIMB OR KNEE SALVAGE IN ACUTE LOWER LIMB ISCHEMIA: A NEW PROTOCOL
Embolectomy has long been the gold standard for treating limbs acutely threatened by arterial occlusion. Delayed embolectomy has not been investigated adequately due to the belief that accompanying mortality and morbidity render the case futile. Following our previous experience with late arterial repair for leg or knee salvage I we applied the same principle to limbs threatened with prolo...
متن کاملA Prospective Study on QT Dispersion in Neonates: A New Risk Factor
Background: QT dispersion (QTd) especially after ischemic heart disease and myocardial infarction is a prognostic predictor of mortality, in adult medicine. Objective: This study was undertaken to determine the correlation between QT dispersion and neonatal stress. Neonates were divided into 3 groups: normal term (30 cases), normal preterm (30 cases) and sick neonates (36 cases), born between ...
متن کاملA Geometry Preserving Kernel over Riemannian Manifolds
Abstract- Kernel trick and projection to tangent spaces are two choices for linearizing the data points lying on Riemannian manifolds. These approaches are used to provide the prerequisites for applying standard machine learning methods on Riemannian manifolds. Classical kernels implicitly project data to high dimensional feature space without considering the intrinsic geometry of data points. ...

متن کاملInvestigating the Effect of Joint Geometry of the Gas Tungsten Arc Welding Process on the Residual Stress and Distortion using the Finite Element Method
Although a few models have been proposed for 3D simulation of different welding processes, 2D models are still more effective in design goals, thus more popular due to the short-time analysis. In this research, replacing "time" by the "third dimension of place", the gas tungsten arc welding process was simulated by the finite element method in two dimensions and in a short time with acceptable ...
